STATUTORY PRICE DISCLOSURES FOR VERMONT PRESCRIBERS

This information is intended for Vermont Prescribers. In accordance with Vermont law, Agios is disclosing to Vermont prescribers, the prices of drugs we market as well as the prices of other drugs in the same therapeutic class. 

The prices listed are Average Wholesale Prices (AWP) as established and reported by a third-party publisher, or as calculated from data made available to the public by a third-party publisher. Note that the price paid by consumers may be higher or lower than the prices listed below.

As required by the Vermont Attorney General, the “Other Products” identified on the forms are the other products listed in the same class as the marketed product in the latest version of the American Hospital Formulary System (“AHFS”) Pharmacologic-Therapeutic Classification system.

The listing of other products in the forms does not imply that such other products are interchangeable with the marketed product or have the same safety or efficacy. Please refer to each product’s FDA-approved label and indication for further information.
